Requirements:
Must be an RN with at least one year of clinical nursing experience.
Responsibilities:
• Coordinate medical care, recovery and return-to-work of injured workers.
• This is a work-from-home position requiring daily car travel using personal vehicle. (Mileage/travel expenses reimbursed.)

SCOPE AND RESPONSIBILITIES
• Conduct intake interviews with injured/disabled, limited individual, head trauma and spinal-cord injured persons and their families.
• Coordinate medical/psychological care by developing, initiating, and instituting a rehabilitation plan for the disabled individual, which includes addressing safety issues. Periodically review all plans for effectiveness to assure that desired outcomes and goals are met.
• Case staffing with other relevant rehabilitation professionals in order to provide appropriate and adequate physical care for the injured individual as prescribed by the physician.
• Monitor care being provided and obtain independent medical opinions when indicated to obtain necessary medical clarification, and apprise involved parties of medical status and care being rendered.
• Facilitate case management activities leading to vocational readiness.
• Provide assistance for home modification, vehicle modifications, PT/OT services, medication regimen, procuring orthotic devices, prescribed healthcare equipment, and coordinating home healthcare services.
• Perform employer interviews with employer representatives to ascertain relevant job duties and essential functions of pre-injury jobs; may assist in job restructuring; determine the availability of alternate, modified, and light duty positions with pre-injury employers, as specified by the physician; conduct job analysis (when appropriate).
• Act as liaison, coordinating efforts with the injured individual, physicians, medical and psychological care providers, attorneys, employers and insurance professionals.
• Assist insurance professionals in determining and projecting future care, needs and expenses; provide pertinent information to assist life care plan specialists.
• Adhere to the policies and standards set forth by our URAC accreditation as well as jurisdictional legislative and regulatory requirements.
• May serve on the QI Committee to identify areas of improvement and future quality improvement initiatives.
• May attend meetings with various Hoover departments to coordinate case management efforts.
• Reports directly to a medical services supervisorHoover Nurse Case Manager (RN)
EXPECTED PROFESSIONAL COMPETENCIES:
• Analyze current health status of injured workers to determine nursing care needs
• Professional level of verbal and written communication skills
• Minimal computer competency
• Basic level of understanding of procedural and legislative rules and regulations
