We are building a virtual-first, home-based cardiac recovery solution that provides a
comprehensive set of tools to educate, rehabilitate, and empower heart disease patients to live
the lives they aspire to live and, in doing so, reduce their lifetime all-cause mortality risk by 50%.
By leveraging technology and evidence-based care plans infused into a behavioral
change framework,  our virtual cardiac recovery program reduces barriers to care. It effectively addresses patients’ physical and mental health needs from home. By extending recovery services to eligible patients that hospital outpatient-based programs do not have the capacity for,  allows Health Systems to serve a broader patient base and gain a new profit center. Further, our clinical model has the flexibility to scale across all heart and vascular service lines and beyond, reducing mortality risk for the chronic disease patients who need this care the
most!

About the Role
We are seeking an experienced, Licensed Clinical Social Worker who is passionate about
improving the lives of patients and advancing equitable healthcare access. In this role, the
LCSW will collaborate with a diverse team to conduct assessments, formulate diagnoses,
develop treatment plans, create personalized care plans, and coordinate care within our team and
alongside clinical teams from our partner Health System.

What you’ll do:
Provide weekly virtual psychotherapy (45-50 min per session) to a panel of approx. 25 - 30 patients
Evaluate patient symptoms and risk factors to determine diagnoses and develop
individualized treatment plans
Perform initial patient diagnostic evaluation for mental health (or behavioral health)
conditions, including screenings and assessments
Develop treatment plans that are supported by evidence-based therapies, current
literature and standard practices
Ensure that treatment plans are supported by standard practices and/or current literature
Follow the plan of care to accomplish functions within established policies, procedures, and scope of practice
Re-evaluate treatment plans. Review and update treatment plans, as needed, to ensure
safety and maximum patient benefit
Promote patient engagement and successfully achieve patient and program goals
Counsel patients using behavioral change techniques (ex. CBT, mindfulness,
motivational interviewing)                                                                                  Communicate effectively across our integrated clinical team and our partner health
systems to ensure the overall health and well-being of the patient
Provide complete, accurate, and timely documentation of patient sessions

What you’ll bring to the table:
Skills/Traits
An innovative mindset, and willingness to challenge the status quo of traditional
cardiac rehab experience
Experience working within integrated clinical teams focusing on polychronic
and/or medicare populations is a plus.  A passion for 1:1 therapy and driving patient behavior change
Understand relevant patient educational content and techniques (e.g.,
Meditation, breath work, etc.) used in psychotherapy
Excellent relationship-building and communication skills with the ability to talk to
many different cultural groups
Positive energy, patience and trustworthiness
Effective and empathetic communication skills across phone, video and
asynchronous messaging
Ability to work in an agile start-up environment
A willingness to travel at least once per quarter for in-person team meetings

Education/Certifications/Licenses
A Masters Degree in Social Work
A valid and current Illinois LCSW license (required) (and willingness to obtain
licensure in additional states)
Minimum of 3-5 years post- graduate experience in a medical and/or behavioral
health setting.
