Compensation: $75,000 - $90,000 Annual Salary

Position Description:

We are looking for a dedicated Social Worker for our Skilled Nursing Facility ( SNF ) in Union, New Jersey. This is a full-time, in-person position, Monday - Friday 9am - 5pm

Qualifications of the Social Worker:

• Bachelor’s Degree in Social Work (BSW) required; Master’s Degree in Social Work (MSW) preferred.

• Valid license or certification (e.g., LSW, LMSW, LCSW) in the state of New Jersey.

• 2+ years of experience in skilled nursing, rehabilitation, hospital, or behavioral health settings preferred.

Responsibilities of the Social Worker:

• Conduct Psychosocial Assessments

• Develop and Implement Care Plans

• Coordinate Discharge Planning and Aftercare Services

• Provide Counseling and Emotional Support to Residents and Families

• Facilitate Interdisciplinary Care Plan Meetings
