Position Description:
We are looking for an experienced Social Worker ( SW ) for our Skilled Nursing Facility ( SNF ) in the Staten Island, New York area to assist residents who are struggling with mental health issues. This is a full time, in person position Monday - Friday 9am - 5pm.
Qualifications of the Social Worker ( SW ) :
Valid Social Worker ( SW ) license in New York
2+ years of experience as a Social Worker ( SW )
2+ years of experience working in a post acute medical setting
Responsibilities of the Social Worker ( SW ) :
Assess patients through sessions and medical history reviews
Develop comprehensive treatment plans
Maintain complete and confidential medical records
