Compensation: $110,000 - $125,000 Annual Salary

Position Description for Nurse Practitioner ( NP ) for our Long Term Care Facilities ( LTC );

We are seeking a dedicated and compassionate Nurse Practitioner (NP) to provide bedside patient care at our Long-Term Care (LTC) facilities in Wall Township, NJ. This is a full-time, in-person role, Monday through Friday, from 9:00 AM to 5:00 PM.

Qualifications of the Nurse Practitioner ( NP ) for our Long Term Care Facilities ( LTC ):

• Valid Nurse Practitioner ( NP ) license in the state of New Jersey
  

Responsibilities of the Nurse Practitioner ( NP for our Long Term Care Facilities ( LTC ):

• Upon admission to the facility, examine residents and their medical records.
• Check ups, management of chronic conditions and medications, transitional care management, discharge planning and family engagement.
