Compensation: $75,000 - $85,000 Annual Salary

Position Description:

We are looking for a dedicated MDS Coordinator for our Skilled Nursing Facility ( SNF ) in Piketon, Ohio. This is a full-time, in-person position, Monday - Friday 9am - 5pm

Qualifications of the MDS Coordinator:

• Valid Registered Nurse (RN) license in Ohio, in good standing.

• 2+ years of experience in Skilled Nursing Facility ( SNF ) or Long-Term Care ( LTC ); prior MDS experience strongly preferred.

• Knowledge of MDS 3.0, PDPM, and EMR platforms (e.g., PointClickCare); RAC-CT certification preferred.

Responsibilities of the MDS Coordinator:

• Complete and manage timely MDS assessments to ensure compliance with federal and state regulations.

• Collaborate with interdisciplinary teams to create and update individualized care plans based on resident assessments.

• Audit documentation to ensure accuracy for PDPM reimbursement and quality measures.
