Compensation:$55,000 - $65,000 Annual Salary

Position Description:
We are looking for a dedicated and compassionate Social Worker for our Skilled Nursing Facility ( SNF ) in Beavercreek, Ohio. This is a full-time, in-person position, Monday through Friday from 9am- 5pm.
Qualifications of the Social Worker:
Active LSW license.in Ohio is required
Experience in long-term care, healthcare, or a similar setting is required.
Strong interpersonal and communication skills with a passion for helping others.
Knowledge of state and federal regulations related to social work in nursing homes.
Responsibilities of the Social Worker:
Provide social work services to residents and their families, offering emotional support and guidance.
Conduct assessments to identify resident needs and develop individualized care plans.
Assist residents in accessing community resources, benefits, and services.
Collaborate with interdisciplinary teams to ensure comprehensive care.
Address and resolve concerns related to resident care and social needs.
Maintain accurate and timely documentation in compliance with state and federal regulations.
