Compensation: $125,000 - $140,000 Annual Salary

Position Description:

We are looking for a dedicated Field Nurse Supervisor for our client in Borough Park NY to oversee caregiver scheduling and ensure seamless coordination of patient services. This role requires strong organisational skills, attention to detail, and the ability to communicate effectively with families, caregivers, and internal teams to support smooth day-to-day operations. This is a full-time, in-person position Monday - Thursday  9am - 5pm, Friday 9am - 1pm

Qualifications of the Field Nurse Supervisor:

• Registered Nurse (RN) license in NYS (required)
• Minimum 2 years of homecare experience (CHHA preferred)
• Prior experience supervising nurses or acting as a team lead.
• Strong working knowledge of OASIS, HHAeXchange, and EVV systems
• Excellent interpersonal, leadership, and communication skills 
• Culturally sensitive and familiar with the Borough Park community (a plus)

Responsibilities of the Scheduling Field Nurse Supervisor:

• Supervise a team of five field RNs providing skilled nursing care to patients at home.Review and approve OASIS assessments, re-certifications, and discharges.
• Conduct routine chart audits and field visits to ensure quality and compliance.
• Train, mentor, and provide clinical guidance to field nurses.
• Collaborate with intake, QA, and scheduling teams to optimise care delivery.
• Act as a liaison between patients, caregivers, and the agency.
• Ensure adherence to DOH, CHHA, and EVV regulatory and documentation standards.
• Participate in case conferences and oversee timely resolution of nursing concerns.
