Position Description:
We are looking for a dedicated Nurse Practitioner ( NP ) for our Home Health Care Agency in Elkins Park, Pennsylvania. This is a full-time, in-person position, 9am - 5pm
Qualifications of the Nurse Practitioner ( NP ):
Valid Nurse Practitioner ( NP ) license in the state of Pennsylvania.
1+ years of experience in Home Care setting.
Required access to a car.
Required travelling to patient's home.
Responsibilities of the Nurse Practitioner ( NP ) :
Examine patients and their medical records.
Check ups, management of chronic conditions and medications, transitional care management, family engagement.
