Compensation: $55,000 - $65,000 Annual Salary

Position Description:

We are looking for a dedicated Social Services Director for our Skilled Nursing Facility ( SNF ) in Bellbrook, Ohio. This is a full-time, in-person position, Monday - Friday 9am - 5pm.

Qualifications of the Social Services Director: :

• 2+ years of experience in a Skilled Nursing Facility ( SNF ) or healthcare setting, with knowledge of SNF regulations and discharge planning.

• Ability to interact compassionately with residents and collaborate with interdisciplinary teams

Responsibilities of the Social Services Director:

• Support residents and families during transitions, ensuring smooth admissions and appropriate discharge.

• Offer emotional support, counseling, and crisis intervention to residents and families, helping them cope with illness, aging, and loss of independence.

• Maintain accurate documentation and uphold compliance with state and federal guidelines, including resident rights, care planning, and social service assessments.
