Job Description:

As a Certified Rehabilitation Counselor specializing in Independent Living Skills Training Services (ILST), you will have a pivotal role in helping waiver participants regain and enhance their ability to live as independently as possible in their communities. This position involves conducting assessments, developing individualized training plans, and providing hands-on training to help participants achieve their goals.

Key Responsibilities:

  • Conduct comprehensive functional assessments of waiver participants to identify strengths and weaknesses related to Activities of Daily Living (ADL) and Instrumental Activities of Daily Living (IADL).

  • Develop individualized ILST Detailed Plans that outline specific goals, milestones, and interventions tailored to each participant's needs.

  • Provide hands-on training and supervision to help individuals with self-care, medication management, task completion, communication skills, interpersonal skills, and other essential life skills.

  • Support participants in acquiring or maintaining skills related to mobility, community transportation, money management, and household maintenance.

  • Assess and address maladaptive behaviors and provide problem-solving skills training.

  • Create a positive and supportive learning environment that maximizes participants' learning potential.

  • Ensure that ILST services are provided in the most effective environment for each participant, whether in their own homes or community settings.

Qualifications:

  • Certification as a Licensed Master Social Worker

  • Knowledge of ADL and IADL assessment and training.

  • Strong interpersonal and communication skills.

  • Empathy and patience in working with individuals facing disabilities.

  • Problem-solving skills and the ability to adapt training approaches to individual needs.
