Job Description: As a Service Coordinator with eligibility for Licensed Master Social Worker, you will play a pivotal role in assisting waiver applicants/participants in gaining access to vital services that enable them to return to or remain in the community. This position involves collaborating with individuals to develop individualized service plans (SPs) that align with their goals and needs. Key Responsibilities: Collaborate with waiver applicants/participants to identify their goals and needs for accessing services. Assist participants in developing individualized service plans (SPs) that reflect their goals and preferences, ensuring they have a voice in decision-making. Coordinate the implementation of approved SPs, working closely with individual service providers and other stakeholders. Regularly review the effectiveness of the SP and make necessary adjustments to achieve desired outcomes. Empower participants to become effective self-advocates and problem solvers, fostering their independence and control over life situations. Support participants in completing the Plan of Protective Oversight (PPO) when required (refer to Appendix C – form C. 4). Maintain up-to-date knowledge of waiver and Medicaid State Plan services, as well as other available resources and programs. Qualifications: A degree in Licensed Master Social Worker Strong interpersonal and communication skills. Ability to collaborate effectively with diverse individuals and service providers. Empathetic and client-centered approach to service coordination. Problem-solving skills and a commitment to promoting independence.
