Provide treatment and intervention for clients within the program to promote optimum mental and emotional health.
Your Responsibilities:
Demonstrates ability to establish rapport with patient and family.
Help patient identify problems, concerns, and develop solutions.
Assist patients in identifying short-term goals.
Teaches and educates patients new coping/adaptive skills.
Establishes realistic treatment goals and expectations.
Demonstrates ongoing evaluation of patient psychiatric issues, psycho-social stressors,strengths/weaknesses.
Structures group therapy so that patients receive maximum benefit.
Provide specific methods to help patients develop interpersonal skills.
Promotes a safe, healing environment for patients.
Encourages patients to reduce isolation through sharing and learning from others
Help patients discuss and alleviate issues related to aging, illness, loss, etc. Provides information education, and support related to the issues of the patient.
Completes psycho-social assessment in a timely and thorough manner.
Demonstrates specific, measurable goals that are realistic to patient's functioning level, continuity between goals and interventions.
Documents progress/lack of toward short-term goals and justifies either discharge or continued treatment.
Provide continuity for treatment goals and planned interventions. Document patient problems, behaviors and progress toward goals.
Assists family and patient in recognizing changing roles and identifying resources as needed.
Ensures topics match interventions identified and assist patients in resolving short-term goals.
Demonstrates competency in leading group sessions on appropriate topics.
Demonstrates timely and accurate assessment of patients discharge needs.
Clearly documents discharge planning with the patient and family.
