Treatment and Triage RN

The Treatment and Triage Registered Nurse will focus on the clinical care of patients for medical office, a Chronic Care Patient Centered Medical Home. The Registered Nurse will primarily work in the treatment and infusion room, taking lead with clinical nursing tasks for complex, chronically ill adult patients in an outpatient environment. The Treatment and Triage Nurse will also coordinate with the Transitional Care Management Nurse to schedule patients in the clinic who need a continuation of skilled, clinical nursing care upon discharge from the hospital or emergency room.

Duties and Responsibilities

  • Infusions
  • Wound Care
  • Medication Administration
  • Inhalation Therapy
  • Inventory and ordering medication and medical supplies
  • Triage patient calls per company policy and procedures
  • Triage “walk-ins” to determine type of visit needed
  • Oversee compliance with quality assurance for point of care testing
  • Facilitate work flows that off-set clinical duties from Nurse Care Managers
  • Assist Medical Assistants who are experiencing a heavy workload, when possible
  • Assist with training Medical Assistants on clinical tasks within their scope and take lead with assessing annual skills competencies
  • Review medical records to evaluate Medical Assistants’ documentation in HER
  • Take lead with rapid response interventions and staff drills
  • Communicates with the multidisciplinary care team and assists with resolving barriers to care and nursing services
  • Provides feedback to management about work processes and helps to modify processes to achieve outcomes.
  • Cross-train in care management activities to allow for coverage in the absence of a Nurse Care Manager
  • Other duties as assigned

Qualifications

  • Registered Nurse with 5 + years related experience in hospital or ED care of complex adult patients with multiple comorbidities
