RN Hospice Case Manager: Boston, MA 

$7,500 Sign-On Bonus

 

An established VNA and Hospice program in the Boston area is currently seeking an experienced RN to be a Hospice Case Manager. This position will provide end-of-life care to patients located in both the facility setting (Senior Living Communities) and within the home care setting.

This is a fantastic opportunity to work with a very resident focused non profit hospice program with excellent leadership and strong community relationships. This position will have a very manageable caseload and excellent resources available to them.

The ideal candidate will be:

• A RN with End-of-Life experience
• A compassionate nurse with excellent bed-side manner
• Ability to travel within the Boston area (Newton, JP,West Roxbury)
