Registered Nurse – Part Time

The Registered Nurse is responsible for initial patient assessments, creation of the patient care plan, and coordination of follow up assessments.
Qualifications:
- Must be a graduate of an accredited college or university
- Must be licensed in the State as a Registered Nurse
- Must have a minimum of one year of experience in home health agency or in a hospital setting
- Must have knowledge of Medicare and Medicaid guidelines
- Should be skillful in organization and in the principles and techniques of professional nursing and required documentation that pertains to it
- Should be skillful in organization and in the principles of time management and have knowledge of management processes.
- Must be able to contribute to the quality of care being rendered through constructive communication with nursing managers and staff
Duties:
- Coordinates total patient care by monitoring the client’s conditions, promoting sound preventive practices, and coordinating services
- Performs admission, transfer, re-certification, resumption of care and discharge OASIS for the home care patient
- Consults with the attending physician concerning alterations of Patient Care Plans, checks with appropriate supervisor and makes changes, as appropriate
- Conducts initial and ongoing comprehensive assessment of the client’s needs
- Coordinates patient services
- Submits clinical notes and progress notes and other clinical record forms outlining the services rendered as indicated
- Discuss with the appropriate supervisor the need for the involvement of other members of the health team
- Cooperates with other agencies to provide continuity of care
- Continually strives to improve his/her nursing skills by attending in-service education, through formal education, workshops, and conferences.
- Adheres to all Federal, state and accreditation requirements including Medicare and Medicaid regulations
Pay is based on per client assessment case which includes:
1. Perform initial patient assessment
2. Create the patient care plan for the CNA
3. Schedule the 90 day reassessment
**Pay is $75-$100 per case**
