We are seeking an experienced Clinical Pharmacist for a 2-month contract assignment in Montgomery, AL. This role is ideal for a retired or semi-retired pharmacist interested in a meaningful, short-term opportunity to apply their expertise in a supportive healthcare environment. The Clinical Pharmacist will provide comprehensive medication and disease state management as part of an interdisciplinary care team.

This assignment includes flexible scheduling and the chance to make an immediate impact on patient care.

Key Responsibilities:

• Conduct comprehensive medication assessments (initial, quarterly, and as clinically indicated), ensuring a patient-centered approach that supports adherence and clinical outcomes.

• Provide disease state management and prescribe medications and treatments within the scope of practice, in accordance with VHA Handbook 1108.11 or subsequent policies.

• Identify and resolve patient-specific medication issues, including drug interactions, side effects, appropriateness of therapy, and adherence barriers.

• Document clinical interventions and therapeutic recommendations accurately and promptly in the electronic health record (EHR).

• Participate in interdisciplinary team meetings, contributing pharmacy expertise to support holistic patient care.

• Serve as a preceptor for pharmacy residents rotating through the HBPC program, providing guidance and oversight.

• Offer educational sessions to pharmacy staff and clinical team members as needed.

• Support additional pharmacy operations or special projects as directed by the Chief of Pharmacy Services.

Requirements:

• U.S. Citizenship required

• Must pass background/security investigation

• Subject to drug screening

• Proficient in spoken and written English

• Must complete onboarding requirements

Qualifications:

• Graduate of an ACPE-accredited College or School of Pharmacy with a BS in Pharmacy or Doctor of Pharmacy (PharmD). (Transcript required)

• Current, unrestricted license to practice pharmacy in any U.S. state or territory. (Copy of license required)

• Prior experience in clinical pharmacy practice, including medication therapy management, medication reconciliation, and patient education.

 

Preferred Experience and Skills:

• Strong verbal and written communication skills, with the ability to influence clinical and administrative decisions.

• Expert knowledge of clinical pharmacy standards and quality assurance practices.

• Proven ability to organize, prioritize, and solve problems in a clinical setting.

• Familiarity with EHR systems and clinical documentation standards.
