Responsibilities

• Monday – Friday (8:00am – 5:00pm)
• Provides case management inclusive of social assistance, including medical and behavioral health coordination to patients
• Working with the refugee population
• Will carry a partial panel of patients for navigation/ care coordination, and will supervise the two bilingual Patient Navigators, confirming that regularly scheduled check-ins occur with patients, and that patients are appropriately and efficiently referred to partnering agencies/community-based resources as needed

Requirements

• Bachelor of Social Work or Masters of Social Work required
• Social Work and Case Management experience required
