Nurse Liaison (Palliative Care)

Our client is a progressive, well-run, and financially stable Hospice company.

Salary and Benefits:

• Annual Salary Range: $105,000 to $115,000
• Health Insurance Plan

• PTO: 24.5 Days Accrued
• 403 (B) w/ Employer Match
• Gas Mileage Reimbursement: 62.5 Cents Per Mile
• Tuition Assistance to Obtain Bachelor’s Degree

Responsibilities:

• Monday through Friday: 8:30 AM to 4:30 PM
• Build Out Palliative Care Referral Base
• Oversee Care Coordination
• Work Closely with the Palliative Care Physician and Chief Medical Officer to Identify, Develop, and Implement Opportunities for Additional or Improved Services
• Coverage Area: Hyde Park, Poughkeepsie, Wappinger Falls, and Pleasant Valley.

Requirements:

• Registered Nurse in the State of New York
• Two to Three Years of Hospice Experience
• Ability to Engage Community
• Hospice or Home Health Experience Preferred
• Reliable Transportation and Active Driver’s License
