Salary and Benefits

• Hourly Rate - $35.00 - $39.00
• $7,500 Sign-on Bonus Subject to change
• Relocation is Case By Case
• Shift Differ: Evenings- $2.00 / Evenings $4.25 (To be eligible for evening/night diff’s, must work 4 hours into the shift. Example: evening is 3p-11p, so one would need to work past 7pm in order to get evening diff.)

Responsibilities

• Implement and monitor patient care plans.
• Monitor, record, and communicate patient condition as appropriate
• Serve as a primary coordinator of all disciplines for well-coordinated patient care.
• Note and carry out physician and nursing orders.
• Assess and coordinate patient’s discharge planning needs with members of the healthcare team.
• Associates or diploma in nursing required; BSN preferred
• Florida RN license required; or appropriate compact
• BCLS–AHA Healthcare Provider certification required within 30 days of employment start date
• National Case Management Certification (CCM or ACM) preferred
• Interqual competence required, certification preferred

Requirements

• Associates or diploma in nursing required; BSN preferred
• Florida RN license required; or appropriate compact
• BCLS–AHA Healthcare Provider certification required within 30 days of employment start date
• National Case Management Certification (CCM or ACM) preferred
• Interqual competence required, certification preferred] Prior hospital case management experience preferred.
• All candidates must have a minimum of 3 years of acute care hospital direct patient care nursing experience in order to be considered.
